2/6/2024 EnchantaVaughn Approved Behavior: Upon joining QMHP, client was observed in the home environment conducting herself appropriately. Client spoke in a polite way when acknowledging QMHP support. Client demonstrates she is capable of paying attention in session without exhibiting attention-seeking behaviors. Client continues to display appropriate social skills in therapeutic setting. Mother reports when she cannot have her way, she will present aggressive behaviors with behaviors lasting up to 10 minutes in duration. 2. Behavior: Client was supported for displaying active listening skills and attending to her work. The client has become less defensive, due to interventions. Client was able to cope and relax in her environment and complete her assignment. Client communicated positive attitude and engaged in activity without frustration or being distracted by external stimuli. Mother reports behaviors can last up to 10 minutes in duration. 1. Intervention: QMHP praised client for her positive conduct and waiting quietly for instructions. QMHP applauded client for the behavior that she was displaying. QMHP complimented client on her outfit, clean face, and fresh braids and display of personal hygiene practice. QMHP coached client to acknowledge and list her strengths, resilience and the positive outcomes that often occur when she makes informed decisions opposed to impulsive ones. QMHP processed client positive behaviors. QMHP reviewed several strategies for conflict resolution and positive social behaviors with client in efforts to increase client’s awareness. 2. Intervention: QMHP assisted client in understanding where she is mentally with her goals versus where she should be mentally with her goals. QMHP practiced calming techniques with client that can be used in moments of high anxiety. QMHP commended client on her improvement and doing a better job at home with her mom. QMHP urged mother and client to participate in role reversal activity to aid mother and client to realize how to respond to and interact with each other. QMHP observed client’s behaviors as seemingly calm and controlled as demonstrated by her ability to release her tension appropriately without feeling misjudged and overlooked. 1. Response: Client was attentive and open with counselor about her feelings and how to identify them. Client discussed mental illness and its causes with mom and QMHP. Client displayed interest in learning about ADHD. Client reviewed ISP objectives as a reminder to stay focus and have faith. Client said she was aware of her expectations and what she is expected to do each day. Client practiced some of the examples of alternate behaviors (e.g. listening to music, going for a walk, coloring or drawing in her journal, pray or talk to someone, repeat positive “I-statements” and ask questions in front of a mirror). Client participated in reviewing key concepts. 2. Response: Client was acceptive of support and encouragement as she was able to reflect openly with QMHP. Client exhibited good social behaviors and did not require redirection to stay on task. Client was able to reevaluate herself and display a more positive attitude toward peers and authority. Client has become more involved in her therapy and was given positive reinforcement for this. Client demonstrated expectations and responded well to feedback. Client communicated positive affirmations and self-talk when expressing herself and responding to the interventions. Client shared some of the things she does at home to help her relax.
